PCDC brings its expertise in data-driven operations, care team design and financial sustainability to Medi-Cal transformation.

PCDC hit the ground running with CalAIM implementation shortly after the initiative launched in January 2022. To ensure high-quality implementation of Enhanced Care Management (ECM) and Community Supports (CS), PCDC has designed services focused on expanding the skills and infrastructure necessary for success in ECM and CS, such as data management and optimization, care coordination, and revenue cycle.

PCDC has worked in California since 2006 (and have staff in Southern California), supporting more than 120 sites with capacity building and enabling more than $35 million in new investment in primary care organizations.  PCDC has supported organizations through previous California initiatives, including the series “Building Care Solutions for Older Adults with Complex Care Needs,” which provided training across the state; the delivery of a six-part curriculum and white paper to California providers on Adverse Childhood Experiences screening (ACEs) and trauma-informed care for Medicare and MediCal beneficiaries; and serving as a trainer for health centers in Community Clinic Association of Los Angeles County’s first CalAIM training cohort in 2022.
