The integration of primary care and behavioral health services is increasingly recognized as a critical component of whole-person care and improved health outcomes. Seamless referrals between primary care and behavioral health providers allow patients to quickly and easily access the physical and mental care they need through an established, trusting patient-provider relationship.

PCDC is prioritizing projects that advance behavioral health integration and services. Recent financings include fully integrated centers and substance use and recovery clinics. Our capacity building team empowers providers to improve care delivery by incorporating behavioral health strategies into existing services.
