PCDC developed this toolkit to assist practices in determining the costs associated with sustaining the tasks of being a Patient-Centered Medical Home. PCMH practices put extra resources into care management, care coordination, and quality improvement initiatives. These extra resources come with extra costs. This spreadsheet-based toolkit helps to identify and calculate the costs. Sustainability is examined across three different areas: process, performance, and outcomes. PCDC developed the toolkit with funding from the New York Community Trust.
