This tool, created by PCDC, can help DSRIP Performing Provider Systems and technical assistance providers assess a primary care practice’s readiness to participate in a DSRIP project. It is in the form of an interview guide.

DSRIP is the Delivery System Reform Incentive Program, active in numerous states, including New York. Led by New York State Medicaid, DSRIP will transform the healthcare delivery system in New York. DSRIP enables Performing Provider Systems (PPS’s) to lead transformation efforts among their cohort/participants.
