Integrated Care in Action: Supporting Whole-person Health and Recovery for People Experiencing Homelessness

Individuals experiencing homelessness often face fragmented care, overlapping behavioral and physical health needs, and significant barriers to consistent treatment. This webinar explores how integrated care can close those gaps and support recovery.

Drawing on the work of the Community Health Center of the New River Valley, presenters will walk through models of integrated care and the policies and frameworks that reduce barriers for unhoused patients, including housing-barrier processes, onsite supports, and care delivered directly in the community. The session takes a deeper dive into recovery options, including office-based addiction treatment (OBAT), wraparound services, peer support, and partnerships with community organizations. Attendees will leave with practical, replicable strategies for delivering whole-person care to one of the most underserved populations.

Webinar Objectives:

  1. Explain how integrated care can support whole-person health, treatment, and recovery for people experiencing homelessness.
  2. Identify integrated systems, practices, and care processes that increase access to care for people experiencing homelessness.
  3. Describe how office-based addiction treatment (OBAT), wraparound supports, peer support, and community partnership can be aligned to support integrated care and recovery.
