Strengthening Primary Care in Rural New York

New York State Rural Health Transformation Program: Funding and support to strengthen your practice so you can keep caring for your community

You know your patients and your practice. But it can be hard to find the time, money, or extra hands to make needed changes to your practice while you’re busy caring for patients.

Get the support you need.

New York State has selected Primary Care Development Corporation (PCDC) to help rural primary care practices through the state’s Rural Health Transformation Program (RHTP).

PCDC brings decades of experience working with primary care providers across New York, including in rural communities. And starting in early 2027, we’ll provide that support to rural providers through the State’s initiative.

PCDC will provide funds for certain capital improvements and technical assistance to help you improve quality, finances, and operations.

Start with something you want to improve in your practice or for your patients

Find and use available RHTP grant funding.

Through the rural health program, we’ll make funding available for certain accessibility and technology improvements.

Improve your office and patient accessibility.

We’ll help you identify accessibility needs and improvements that make it easier for patients to receive care. That may include physical improvements or accessible medical equipment.

Strengthen the business side of your practice.

We’ll advise you on practical ways to increase reimbursement in your practice, focusing on the revenue cycle and mitigating the impact of the coming Medicaid changes.

Strengthen operations for improved quality and revenue.

We’ll help you secure additional revenue from New York State by meeting state-recognized primary care standards (Patient-Centered Medical Home) or participating in broader Medicaid programming that can qualify you for enhanced payments. We can help determine whether that makes sense and guide you through the process.

Use technology when it is useful.

Implementing the right technology can make it easier for you to care for your patients and accomplish your quality and revenue goals. We can weigh the pros and cons of a particular solution, investigate whether it can reduce work or solve a real problem, and guide you through putting it in place.

Why PCDC?

For more than 30 years, PCDC has worked with primary care providers to strengthen practices and expand access to care. PCDC has experience working with primary care providers across New York, including rural communities. New York State selected PCDC for this work because of our primary care experience and deep advocacy for the field.

“PCDC has uniquely deep primary care expertise, are strong advocates for primary care, and understand the challenges facing rural health care organizations. Our experience with them demonstrated their commitment to rural primary care providers. Their approach builds trust. Rural practices need partners who understand that transformation must strengthen—not disrupt—the relationships and systems that communities already rely upon.” Said Michael C. Pease, President and CEO The Chautauqua Center, Dunkirk, NY
