PCDC’s Webinar Series Know Before You Enroll – Making Care Primary
CMS announced a new value-based model from The Centers for Medicare and Medicaid (CMMI) – “Making Care Primary” (MCP). This new model underscores federal policies around health equity, health-related social needs, and alternative payment models for primary care providers. Applications are now open in eight pilot states (Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina, and Washington) to Federally Qualified Health Centers (FQHCs) or independent primary care practices interested in Value-Based Care.

That is why we created a three-part webinar series: “Know Before You Enroll – Making Care Primary” to break down MCP into simple components. The first is specific to FQHCs’ needs, the second is tailored to independent practices, and the third is to discuss any unanswered questions.
