PCDC’s Webinar Series Know Before You Enroll – Making Care Primary
CMS announced a new value-based model from The Centers for Medicare and Medicaid (CMMI) – “Making Care Primary” (MCP). This new model underscores federal policies around health equity, health-related social needs, and alternative payment models for primary care providers. Applications are now open in eight pilot states (Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina, and Washington) to Federally Qualified Health Centers (FQHCs) or independent primary care practices interested in Value-Based Care.
We are experts who partner with health centers and primary care practices to transform and sustain their operations. We recognize that MCP is a comprehensive and complex program. That is why we created a three-part webinar series: “Know Before You Enroll – Making Care Primary” to break down MCP into simple components. The first is specific to FQHCs’ needs, the second is tailored to independent practices, and the third is to discuss any unanswered questions.

Expanding and Enhancing Primary Care: A Pathway to COVID-19 Recovery
Primary care is the foundation of a high-functioning, high-quality and equitable health care system. Research shows that increased access to primary care is associated with fewer hospital visits, emergency department visits, and surgeries. Yet, primary care represents only an estimated 5–7% of health care spending in the United States.
At the same time, the neighborhoods hardest hit by COVID-19 – those with a higher proportion of patients of color and low-income residents and higher rates of chronic disease – are the same neighborhoods that have poor access to primary care. The need for primary care is expected to increase significantly following the pandemic, particularly in the communities with disparities in acute and chronic illness.

June 5, 2021 marked the 40 year anniversary of CDC’s Morbidity and Mortality Weekly Report (MMWR) describing the first cases of a rare pneumonia among five men in Los Angeles, marking the first official reporting of what would later become known as HIV/AIDS. Since then, we have lost 32 million to the virus and 38 million are currently living with HIV globally.
Since 1981, we’ve witnessed many advancements in medicine and technology and today, the end of this pandemic is in sight. However, we’ve recently had to put our creativity and innovation to the test— while we are in the midst of ending HIV we were caught off guard with COVID-19. As a result, many health departments, community based organizations and health centers had to identify new ways to provide HIV prevention services, care and treatment. We will not give up on the fight. We now KNOW how effective HIV testing, PrEP/PEP and ART can be in eliminating new HIV infections, but we must continue digging away at stigma and the social and structural barriers that keep people from engaging in care. We must recommit ourselves to eliminating racism, homophobia, and transphobia in health care, and move towards health equity for all.

Improving Status-Neutral, Gender-Affirming, Integrated Services for Transgender People
The Primary Care Development Corporation (PCDC) hosted the launch of the virtual Northeast Region Community of Practice (CoP). This CoP brought together experts who have lived experience and/or members of the community working to increase transgender health equity and gender affirming care. Participants engaged in a collective learning process which will include sharing knowledge, lessons learned, best practices, and resources to improve health care and HIV status neutral services for transgender communities.
At this session we covered:
- Discuss innovative practices on how to improve services for transgender persons
- Brainstorm ideas for future community of practice sessions
- Identify the needs of participants and discuss about how to make the community of practice most useful for your organization

The new COVID-19 Community Health Care Resiliency Fund provides flexible, low-cost working capital loans and customized technical assistance to community health centers in New York State. PCDC Virtual Town Hall: The Impact of COVID-19 on Health Care Providers and Organizations
Watch Primary Care Development Corporation (PCDC)’s 90-minute June 23, 2020 Virtual Town Hall on the impact of COVID-19 on health care providers and organizations. This global crisis has placed an unprecedented strain on our systems of care. Each day, providers (including all levels of health care staff), are asked to manage challenges and develop creative solutions for providing continuity of care in the context of the pandemic. The Town Hall provided a forum for providers to share information and ideas for making complex decisions, integrating rapidly changing clinical guidelines, exploring the impact of health disparities on health outcomes, addressing staff burnout, and sustaining health center operations.

With financial support from the New York City Council, the Primary Care Development Corporation (PCDC) produced an FY21 report, Primary Care Access and Equity in New York’s City Council Districts.
PCDC’s Evaluation and Analytics team reviewed key findings within the report, including the relationship between access to primary care and the severe impact of COVID-19 across the city. PCDC’s Policy and Advocacy team discussed several policy recommendations for Council Members, policy makers, and primary care advocates to consider to increase primary care access and equity across the city.

PCDC released its new data brief indicating that better availability of primary care in a New York City neighborhood correlates with better health outcomes (including COVID-19 outcomes), with district-by-district information to help inform New York City Council Members and staff. PCDC’s Evaluation and Analytics team also provided an overview of PCDC’s Primary Care Safety-net Access project and free resources on primary care access across New York City. PCDC also shared information about a new partnership with the Wells Fargo Foundation, investing in primary care workforce development programs to encourage people from diverse backgrounds to seek training and provide care in their communities.

Primary care saves lives, leads to improved individual and community health, and is unequivocally central to health equity. Primary care is the only part of the health system that has been proven to lengthen lives and reduce population level health disparities and reduces overall health care costs. Yet, we continue to undervalue and underfund it, and the effects are felt most acutely by marginalized communities.
Pending legislation in the New York State legislature would initiate the first step in a transformational process to reorient New York’s health system towards primary care, encouraging investment in the care that works best for people and communities. A7230A (Gottfried)/S6534B (Rivera) would convene a collaborative of expert stakeholders to gather data, determine the best path towards centering primary care and make recommendations to the legislature to increase the proportion of health care spending that goes to primary care in New York State.
The Primary Care Development Corporation (PCDC) hosted a Legislative Briefing with legislative cosponsors Assemblyman Richard Gottfried and Senator Gustavo Rivera, and PCDC CEO, Louise Cohen, as they shared critical information about how primary care matters both as a response to COVID and beyond, how investing in primary care helps communities and is essential to health equity, and why A7230A/S6534B is a critical first step towards increasing access to primary care for all New Yorkers.
The event also featured Dr. John Rugge, Founder of Hudson Headwaters Health Network and Dr. Pascale Kersaint, Chief Medical Officer at Brooklyn Plaza Medical Center as they shared their perspectives on how primary care providers make a difference in their communities, challenges providers face, and how we can support them and their communities better by investing in primary care.

The U.S. Health Resources and Services Administration (HRSA) recently announced new rounds of funding for Federally Qualified Health Centers (FQHCs) and provider organizations with $1 billion being allocated for HRSA-Health Center Program funded FQHCs to support major construction and renovation projects. Additionally, $150 million was recently allocated for FQHC Look-Alikes (LALs) to support COVID-19 mitigation efforts, including vaccinations, testing, and other health services. View the official announcement impacting LALs.
PCDC and Advocates for Human Potential hosted a workshop on how to organize your organization’s COVID-19 response and recovery efforts over the next two years. Participants were provided brief overview of the reporting requirements for the HRSA funds, tangible tools to organize their internal planning efforts, and guidance on other complementary sources of capital.

This webinar will feature experts Dr. Anitha Iyer of Mount Sinai Health System, Scott Munro of Headway, and Dr. Andrew Philip of PCDC. Dr. Iyer is the Director of Behavioral Health Population Management at Mount Sinai Health Partners.
