Henrietta Croswell is a Senior Director in the Clinical and Quality Partners team. She joined PCDC in 2012 to support the unit’s Patient-Centered Medical Home and practice facilitation efforts. Today, she oversees a portfolio that includes HIV and sexual and reproductive health; care management and care coordination; training; and behavioral health; she works with clients ranging from AIDS Service Organizations to large health systems and health departments. Prior to PCDC, Henrietta was the Senior Practice Manager for Premier HealthCare’s largest Brooklyn health center dedicated to serving the intellectually and developmentally delayed population. She also supervised the adult day programs at YAI. Throughout her career, Henrietta has overseen the expansion of primary care sites to include ophthalmology, gynecology, podiatry, endocrinology, and behavioral health services and the expansion of site hours to include evenings and weekends. She has also led the successful expansion and reapplication of a CDC HIV Capacity Building Assistance center. Henrietta has a bachelor’s in psychology and a Master of Public Health in community health education from Hunter College in New York City. She also holds a certificate in American Sign Language and is a Patient-Centered Medical Home Certified Content Expert.
