The Primary Care Development Corporation (PCDC) is providing no-cost technical assistance to eligible practices as part of the New York State Patient-Centered Medical Home (NYS PCMH) Recognition Program, a customized PCMH Recognition Program that supports the state’s initiative to advance primary care through the medical home model and promote the Triple Aim: better health, lower costs, and better patient experience. The program is a collaboration between the National Committee for Quality Assurance (NCQA) and the New York State Department of Health (NYSDOH).

As an NYSDOH-contracted transformation assistance provider, PCDC offers step-by-step assistance for practices participating in NYS PCMH, helping them manage the transformation process and improve the patient experience. The technical support is available for a limited time based on availability of funding and practice eligibility.

NYSDOH covers the first year NYS PCMH Recognition fee or the first NYS PCMH Annual Reporting fee for participating practices. Participating practices may also be eligible to receive supplemental payments through state programs such as the Medicaid PCMH Incentive Program.

Since 2008, PCDC’s content experts have provided strategic leadership, practice coaching, and technical assistance to community health centers, hospital outpatient centers, private practices, and special needs providers across the country.

These services recently yielded an important milestone: 450 primary care practice sites achieved National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home recognition following assistance from PCDC.

The PCMH model emphasizes care coordination, population health, evidence-based guidelines, and effective use of health information technology to meet the patient’s needs. Almost 15,000 practice sites — with more than 70,000 clinicians — are recognized by NCQA nationwide.

PCDC’s Performance Improvement team is exclusively comprised of PCMH Certified Content Experts (CCEs). PCDC uses a tailored approach, “meeting practices where they are,” to create individualized plans for improvement, translate complicated standards into attainable tasks, and guide practices through the PCMH recognition process from start to finish.

Throughout the transformation process, PCDC works with practices to tailor work flows and activities to their existing resources. In addition to assisting with PCMH recognition, PCDC coaches guide the transformation of clinical and administrative operations to prepare practices for a value-based payment (VBP) environment, ensuring successful participation in other programs like Medicare’s Merit-Based Incentive Payment System (MIPS).
