PCDC Board member Gary M. Jacobs, MPA, has been appointed President of Village@Home, an extension of the physician-led, team-based platform developed by VillageMD, a national provider of primary care.
In his new role, Jacobs will “lead and accelerate the expansion of Village@Home to physician groups, hospital systems, and health plans nationwide,” according to VillageMD.
“I’m excited to help lead VillageMD’s charge to revitalize in-home care so every patient can access the kind of care they need and the compassion they deserve in the comfort of their home,” Jacobs said in an official statement.
An in-home health care and managed care expert, Jacobs brings more than 25 years of expertise in Medicare Advantage, Medicaid, Medicare Supplement, post-acute care, long-term care, exchanges, and payer-provider collaborations.
Throughout his career he has specialized in identifying new business opportunities, developing long-term growth strategies and strategic alliances, and implementing enterprise-wide initiatives for health plans and post-acute and long-term care providers.
Most recently, he served as Executive Vice President for strategic relationships for CareCentrix, where he advanced CareCentrix’s transition to value-based care through external partnership development.
Earlier, Jacobs was a Managing Director in the Heath Industries Advisory Practice at PricewaterhouseCoopers (PwC), leading the National Government Programs practice.
