The Patient-Centered Medical Home: Taking a Model to Scale in New York State

The Patient-Centered Medical Home (PCMH) is an important innovation that promises to improve health care quality and reduce costs in New York State. The State has embarked on an ambitious plan to enroll one million people in medical homes and health homes through the expansion of multipayer initiatives, the use of public health insurance programs, and enhanced payments for care coordination. This report describes the PCMH model, reports on how the model is being adopted in pilots and demonstrations across New York State, and highlights the policy and pragmatic issues related to the model’s current implementation and the possibility of instituting it on a broader scale.
