This publication provides tips and tools to support the Patient-Centered Medical Home Model of Care in both existing and new health center facilities. It educates health center sta on the PCMH principles and provides available resources for implementation. The publication is presented in two parts: the first section focuses on design and flow interventions for existing facilities that communicate a patient-centered rather than staff- centered environment, and the second focuses on creating new spaces that support care delivery in a medical health home.
