patient centered medical home (PCMH) emphasizes the central role of
primary care and care coordination, with the vision that every person
should have the opportunity to easily access high quality primary care
in a place that is familiar and knowledgeable about their health care
needs.
Increasingly
PCMH demonstration projects have shown significant impact on cost, quality and patient experience (see Resources). Many health plans, including Medicare and Mediciad, are
revising payment models to reward providers that meet PCMH innovations
and have a measurable impact on patient outcomes.
