PCDC Welcomes Gary Jacobs and Carol Raphael to its Board of Directors

(New York - NY) - PCDC has elected two new members to its Board of Directors: Gary Jacobs, MPA, Senior Vice President at Universal American Corporation and Carol Raphael, MPA,
former President & CEO of the Visiting Nurse Service of New York.
Mr. Jacobs and Ms. Raphael join 26 others in contributing to PCDC's
strategic direction and financial oversight.

in planning, finance, governance and health policy that will be critical

Gary Jacobs, MPA, is Senior Vice President for Corporate Development at Universal American Corporation where he overseesthe
company's government affairs and corporate development efforts. During
Mr. Jacob's tenure, Universal American has grown from a $750 million to
a $5.5 billion company, and sold its Medicare Part D division to CVS
Caremark for $1.25 billion in early 2011. Prior to Universal American,
Mr. Jacobs was President & Founder of CHCS Services, acquired by
Universal American in 2000; and Executive Director of Staff Builders
Health Care Services. He currently serves as a board member of the Coral
Springs Museum of Art, the Dean's Advisory Board at American
University's School of Public Affairs, and the executive committee of
the Patient-Centered Primary Care Collaborative (PCPCC). Mr. Jacobs
holds an MPA from American University.
Carol Raphael, MPA, served
as President and Chief Executive Officer of the Visiting Nurse Service
of New York (VNSNY) from 1989 to 2011. Prior to joining VNSNY, Ms.
Raphael held positions as Director of Operations Management at Mt.
Sinai Medical Center and Executive Deputy Commissioner of the Human
Resources Administration in charge of the Medicaid and Public
Assistance programs in New York City. In 2011, Ms. Raphael was appointed
to Governor Andrew Cuomo's Medicaid Redesign Team and served as
co-chair of the Managed Long-Term Care Implementation and Waiver
Redesign Work Group. She chairs the New York eHealth Collaborative, is a
member the National Quality Forum Coordinating Committee, and incoming
Chair of the Long-Term Quality Alliance. Ms. Raphael serves on the
Boards of Directors of AARP, Pace University and the Continuing Care
Leadership Coalition. She is also a member of the advisory boards
for several organizations, including the Harvard School of Public
Health's Health Policy Management Executive Council and the New York
City Department of Health and Mental Hygiene. She holds an MPA from
Harvard University's Kennedy School of Government, and was a Visiting Fellow at the Kings Fund in the United Kingdom.
