Dear Friends,   

 

Primary care is not an island when it comes to improving care and reducing costs; especially for our most vulnerable patients. 

 

Instead, the model we need calls for integrating primary care with mental and behavioral health, specialty care, HIV/AIDS, hospital and long term care services, and community resources. Going forward, responsibility for care will increasingly move toward high-performing, integrated health networks that include the entire spectrum of services -- and most importantly, is built on a foundation of excellent, high-performing primary care.   

 

We are eager to hear from you as you think about and act on these challenges, and hear how PCDC's services might be shaped to help you and other providers achieve these goals. With continued efforts and collaborations, we will continue to see the evolution of a true integration and we look forward to working with all of you!
