Performance Improvement - Medical Home Implementation - New York, NY
PCDC is part of a project funded by the National AIDS Fund and Bristol Myers-Squibb to bring more coordinated and consistent primary care to more than 1,500 New Yorkers living with HIV/AIDS. Called ACCESS NY, the project brings together PCDC, the NYC AIDS Fund; Amida Care, and local HIV/AIDS specialty provider groups.
The partnership combines Amida Care's experience in enrolling and recruiting new HIV+ patients and their extensive HIV primary care provider network with PCDC's expertise in redesigning systems of care to focus on quality, continuity, and patient access. New York is one of five regions nationally to receive the award.
PCDC is implementing a Learning Collaborative with partnering primary care medical organizations and providing technical assistance to maximize real-time access to HIV medical care, improve clinic and provider performance and ultimately facilitate retention.
