PCDC is committed to helping meet the capital needs of community-based health providers across the country to expand and enhance primary care and other vital health services.

We raise capital from investors, government, banks, and foundations nationwide that share our commitment to advancing health equity. All look to us as a leading conduit of change—minimizing cost, maximizing flexibility, and optimizing outcomes.

We are mission-driven financial experts who help health centers and practices to find the right financial solutions to achieve their expansion goals.

We supply essential and flexible financing, especially in communities of color and other disinvested communities, including traditional term financing, short-term and/or bridge financing, tax credit financing, or selective working capital facilities.

New Markets Tax Credits Financing

Our NMTC program provides flexible, low-cost financing for primary care projects in underserved communities.

New York State Revolving Capital Fund

Our public-private partnerships provide new financing avenues for primary care projects across the country.

Transformation Loan Fund

Our Transformation Loan Fund enables health centers and other providers to undertake critical investments that support new delivery and payment models.

Stories of impact

We are a national community development financial institution (CDFI). Our Capital Investment Team provides capital financing, training and technical assistance to increase primary care capacity and advance equity in health.

  • Genesee/Orleans Council on Alcoholism and Substance Abuse (GCASA) offers services and support for those impacted by addiction. After being awarded a grant that required GCASA to finance its project costs upfront, we provided a flexible $1.75 million bridge loan to support the organization during construction.

  • Santa Cruz Community Health Center (SCCHC) is a Federally Qualified Health Center (FQHC) that provides primary care services to patients regardless of their financial status. In 2020, SSCHC reached capacity and needed to construct a larger health clinic. We closed on a $10 million NMTC allocation and a $2.0 million source loan to support the construction.

  • Premier Community HealthCare Group, Inc. is a Federally Qualified Health Center (FQHC) that serves more than 36,000 patients in Florida. In 2019, Premier developed a plan to expand services to their patients. To support the expansion, in collaboration with other CDFIs and CDEs, we provided $16.5 million of NMTC and $1.9 million in loans.

  • Henry J. Austin Health Center (HJAHC) is an FQHC and the largest non-hospital based ambulatory care provider in Trenton, NJ. In 2019, HJAHC decided to renovate their main site to make it more therapeutic. We provided nearly $3 million towards the $4 million project. The facility was recapitalized in 2022.

  • Community Health and Wellness Center of Greater Torrington (CHWC) is a Federally Qualified Health Center (FQHC) in the northwest corner of Connecticut. We offered CHWC a $1 million loan to support its efforts in aligning its care delivery model with a Patient Centered Medical Home (PCMH) model.
