Project Managers, Performance Improvement
Primary Care Development Corporation (PCDC) seeks Project Managers to assist in advancing projects that expand and transform primary care in underserved communities to improve health outcomes, lower health costs and reduce disparities. Project Managers plan, schedule and control all project activities towards the successful execution of assigned milestones and tasks, manage team resources to ensure outcomes are delivered on-time, on budget and within defined scope, oversee timeline and cost parameters, apply technical, theoretical and managerial skills to satisfy project requirements, while building positive professional relationships with clients (primarily primary care providers and community health centers) and associates. Project Managers work directly with our clients to achieve project goals.
As a Project Manager in PCDC’s Performance Improvement Practice, you will:
- Plan and implement quality improvement programs within the primary care setting to ensure performance excellence
- Provide support, education and act as a resource for physicians, staff and others as needed related to quality improvement, safety initiatives and performance excellence
- Facilitate clients through the change management process and redesign of workflows that support the PCMH areas of Access, Care Coordination, Patient-Centered Care and Population Management
- Collect and evaluate data related to clinical and operational performance and medical home transformation activities; identify trends and issues; and facilitate the development of action plans toward medical home transformation and client specific performance improvement
- Coach clients on the effective design and implementation of plans for sustainability and spread enabling practices to effectively continue quality improvement activities
Project Managers report to the Managing Director, Performance Improvement but may be directed on specific assignments by other Directors and Senior Managers within the Practice.
Minimum Qualification Requirements:
- Clinical background highly desired (e.g. MD, RN, LPN, PA)
- Bachelor’s degree, Master’s degree in public health or related discipline preferred
- Knowledge of health information technology in the primary care setting, including implementation of EHR/PMS, design and use of data to improve quality
- Demonstrated experience managing multiple simultaneous projects involving cross-functional product teams within budgetary and schedule constraints
- Demonstrated experience directing internal and/or external project teams
- Demonstrated skills and experience in negotiation, problem identification and resolution
- Strong teamwork, communication and interpersonal skills
